NATA Compliance Services made a few adjustments to the "Specimen Collection Request Form" for 2007.

2. Employer (Contact) Fax No. (added)
3. MRO (CCF) Fax No. (moved to top of form)
4. MRO (ATF) Fax No. (moved to top of form)


The adjustments were made to clarify CCF and ATF distribution instructions for the collection sites. Please print the attachment and use for all future Drug and/or Alcohol collection authorizations.

Each donor should be supplied with the following prior to collection:

1. Chain of Custody (CCF) - Drug Collections ONLY.
2. Specimen Collection Request Form (completed by Employer)


NOTE: Alcohol Testing Forms (ATF) are held at the collection site.
